Wallmann's regime Hello everyone Today we are going to discuss Wallmann'sregime and Alfredson's regime. Which is very useful for the rehabilitation of Achilles tendinopathy. Wallmann's regime After the initial 5-10 min, a warm-up patient will do a calf raise exercise progressed by increasing the load and speed of eccentric lowering. Level 1 Bilateral Straight leg hill raise. Then put the toes down of injured leg down and lower both legs slowly and alternatively. 5 Sec*10 repetition*3 sets Increase the lowering speed to 2 seconds, and to 1 second. Level 2 Do both legs lowering together. Level 3 Progress to the uninjured leg alone during the raising phase and the injured leg alone lowering. Level 4 Progress to both legs during the raising phase and the injured leg alone lowering. Level 5 ...

Achilles tendinopathy The Achilles tendon can be damaged due to overloading during the propulsive phase of the gait cycle. It can be damaged in almost all sports which involve running like marathon running, football, badminton etc. After injury any body part needs rest to recover but in case of Achilles complete rest is not possible, because during simple walking also Achilles tendon is going to work. the Achilles tendon has the poor blood supply, which also one of the reasons for delayed healing. Sign and symptoms Pain over tendon during activity. Sometime pain may be felt after activity because due to reactive loss of sensitivity. Mild swelling over the tendon Treatment Anti-inflammatory medicines Physiotherapy treatment 1. Ice application 2. Modalities 3. Massage 4. Taping 5. Shoe modification 6. Exercise regime. Wallman's and Alfredon regimes are now most commonly used treatment for it. In next article, we will discuss it in detail. For m...

PROPRIOCEPTION What is proprioception? Proprioception is a specialized variation of the sensory modality of touch that encompasses the sensation of joint movement (kinesthesia) and joint position (joint position sense). If we simplify it than proprioception is the the sensation which make us aware about our joint position in space. For example, with closed eye also you can be aware that your elbow is bend or your fingers are open etc. What structures are responsible for these sensations? The sensory receptors for proprioception that are found in the skin, muscles, and joints as well as in ligaments and tendons all provide input to the central nervous system (CNS) regarding tissue deformation. Visual (Eyes) and vestibular centers (Ear) also contribute afferent information to the CNS regarding body position and balance. What happens in case of injury? Trauma to tissues that contain mechanoreceptors may result in partial de-afferentation (Massage cannot be send ...

Medial Tibial Stress Syndrome Medial tibial stress syndrome (MTSS) is a common exercise-induced leg injury among athletes and military personnel . How to identify MTSS? You will feel pa in along the postero-medial border of the tibia that occurs due to exercise…’’ and in addition: ‘‘pain by palpation of the posteromedial border of the tibia is present over a length of five or more consecutive centimeters’’. Pathology Currently, it is thought that MTSS is a bony overload injury; i.e., the tibial bone bends during weight-bearing activities causing strain in the tibia. This strain normally causes micro damage in the bone, which leads to bone adaptation processes to strengthen the bone to resist tibial bending. When this strain exceeds a certain threshold and becomes overloaded, the osteoclast activity may outpace osteoblast activity, leading to local tibial osteopenia. Treatment (According to research articles published after year 2010) 1....
